Focus Care is looking for candidates who may be interested in picking up supplemental income by performing HRA’s. This program has been very successful with our present Health Care Providers this year for several reasons:

This is a Part time position that can be performed around the schedule of your busy day.  You make your own hours, any day of the week. No office hours

·The Nurse Practioners/Physician Assistants decide which zip codes they would like to travel within , and will only be asked to travel outside of that zip code (with added incentives )if no one else is available.

·There is no prescribing of medication and there are no treatments involved with this program. Diagnosing is minimal, Evaluation lasts 45-60 minutes, Annual visit only

·

The Nurse Practioners/Physician Assistants will provide basic proactive behavioral management recommendations, and Case Management referrals when necessary. Focus Care supplies all the equipment, training and 24 hour support. Evaluations are modeled within the Comprehensive Geriatric Assessment format

The greatest part of this position is that you “truly make a difference” in the members lives, we are going into members homes where we can see how a member lives and interacts within their own environment.

Evaluations are not meant to take the place of annual Physicals from PCP’s. PCP’s, however, will receive your recommendations
